It would be valuable under Chronic Conditions to include option to elect more than 1 condition, such as if one is seeking providers caring for heart related diagnoses, that one could elect Atrial Fibrillation, AMI , Heart Failure/CHF. Hyperlipidemia and HTN all together!

It would be valuable under Chronic Conditions to include option to elect more than 1 condition, such as if one is seeking providers caring for heart related diagnoses, that one could elect Atrial Fibrillation, AMI, Heart Failure/CHF, Hyperlipidemia and HTN all together! This is useful when starting new Palliative Care program to focus on heart related disease/illness and providers caring for these unique patients.

    Hi Liz, my thought would be that there would need to be more then one slider bar- each corresponding to separate chronic conditions. But would possibly be more helpful would be to provide the ability to select multiple CCs and add columns to the chart on explore that would then list patient costs with the column headers being the individual CCs.

    Hello Connie,

    Thank you so much for your idea submission! Currently on the filter there is also a slider that let's a user narrow in on count of patients. Would you expect that number to look at one or both of the conditions selected?

    For instance Dr A has 20 CHF and 30 Atrial Fibrillation: would I put 20 on the slider and basically say "both chronic conditions have to be over 20" or would I put 50 and say "the combination of these 2 conditions should be over 50"?
