Per Trella support, at this time we don't have a facility specific readmission rate for a specific home health agency, SNF, or hospice. It would be helpful for us to see ours in relation to specific hospital(s). Also, it would be helpful to see our readmission rates compared to State or County like other facilities on the trend table/chart e.g. see attached email thread with examples and specifics.

Thank you so much for submitting your idea. We have added the Facility Specific Readmission rate within the SNF site under the Sources tab (Inpatient dropdown). I'll include a screenshot for reference. Is this a helpful area to have this information or is there a better table/area to have it added for HHA and HOS?
